Lets Get Started!
In order to get started with speech therapy services, including evaluation and treatment please:
1. Send us a copy of the front and back of the child’s insurance card.
2. Send us a copy of the child’s most recent hearing screening (hearing screening must be less than 6 months old).
3. Send us a copy of current public school Individualized Education Plan (IEP) if applicable.
4. Please complete of the following forms
We will contact you to set up an evaluation once we receive and review the information above.
